Complete Enrollment Packet IN PERSON
Once your student has been selected and accepted to NASA from STEP 2: LOTTERY, the next step is coming to our office and completing an enrollment packet for review by our Admissions team. This will include the following:
- Enrollment form (Provided on arrival)
- Birth certificate
- Immunization record
- PAR/Exit paperwork from previous school
- Latest report card
- Indication of whether the student may require special education or related-services (IEP or 504, if applicable)
- Home language survey to determine and allocate additional resources for English language learners.
- Authorization for school to request/receive student records from all schools student has previously attended, or is currently attending
- Proof of health examinations and oral examination required by the Health and Safety code.
- Utility bill (proof of residence)
- Court paperwork (if applicable)
- CAASP Testing results from the previous year (if applicable)
- GATE (advanced learning / if applicable)
- Green medical form (1st grade only)
